Description

American Traveler is seeking a Physical Therapist for a Home Health assignment, requiring 1+ year of home health experience and active state licensure.

Details

  • Home Health setting providing skilled visits for Medicare, Medicare Advantage, and commercial insurance patients
  • Day shift schedule, 5x8 hours (8:00 AM - 5:00 PM)
  • Patient population includes med/surg, cardiac, ortho (hips/knees/shoulders), post-surgical, COPD, CHF, pneumonia, CVA, falls, weakness, Parkinson's, failure to thrive, respiratory, neuro, trachs, trauma, AKA/BKA, and ADL-focused patients
  • 6-8 home skilled visits per day, with a minimum of 6 points per day (may increase at manager's discretion)
  • 1-2 weekend shifts required per contract
  • Uses BayCare Forms, BayCare Scheduling, and Ace Timesheet documentation systems
  • Laptop, MiFi, and portable printer provided for documentation and connectivity

Requirements

  • Active state PT license required if applicable
  • BLS certification required
  • Minimum 1 year of experience required, including home health experience
  • OASIS training preferred
  • Cerner experience preferred
  • Must have a valid driver's license and reliable vehicle for home visits
  • Proficiency with equipment including gait belts, walkers, sit-to-stand lifts, sliding boards, cervical collars, and wheelchairs required
  • Skilled in ortho, neuro, cardiopulmonary, and documentation practices including evaluations, care plans, and OASIS assessments
  • Must be COVID vaccinated or hold an exemption on file
  • Flu shot or declination required, with mandatory mask wearing at all times while at work if declined

Additional Information

  • Responsibilities include patient assessment, treatment planning, home exercise programs, medication education, and family/caregiver education
  • Must recognize and report signs of physical, sexual, or psychological abuse
  • Focus on safety and injury prevention during home visits
  • Any solid color scrub permitted
  • Locals accepted at the same pay rate
  • Open to candidates without previous travel experience
