Description

American Traveler is seeking a Physical Therapist with home health experience for a 13-week travel contract in a home health setting, requiring Homecare Homebase (HCHB) proficiency.

Details

• Home health care setting serving patients in their homes across an assigned coverage area
• Patient population includes CHF, orthopedic, and pneumonia diagnoses
• Daily point expectation of 6.5 points, with responsibilities including Start of Care (SOC), Resumption of Care (ROC), recertifications, evaluations, and routine visits
• Charting is performed using Homecare Homebase (HCHB)
• Schedule is Monday–Friday, 8am–5pm with rotating weekend shifts (8am–5pm)
• Holiday rotation is required
• 8-hour shift days

Requirements

• Minimum 1 year of physical therapy experience required; new graduates are discouraged from applying
• Home health experience is required
• Proficiency with Homecare Homebase (HCHB) is required
• Active BLS certification (AHA) required; pending certifications will not be accepted
• Active state or compact (NLC) PT license required for the state of placement; pending licenses will not be accepted — license required if applicable
• Two professional references required: one supervisor from within the last 12 months and one peer or supervisor from within the last 3 years, including dates of employment and eligibility for rehire
• A copy of a valid driver's license is required for consideration

Additional Information

• Responsibilities include conducting SOC, ROC, recertification, evaluation, and routine home visits
• A clinical manager or educator is available 24/7 for field emergencies and 8am–5pm for general support
• Orientation includes approximately 2 weeks with a work coach, including supervised field visits; orientation totals 12 hours
• Either scrubs or business casual attire is acceptable
• A phone and tablet will be provided for use during the assignment
