Job Summary
Physical Therapist Home Health The organization is seeking a qualified Physical Therapist specializing in Home Health to provide in-home therapy services. The role involves delivering assessments, developing treatment plans, and supporting patient recovery efforts in a home environment.

Job Details

Certifications Required: BLS, ND Physical Therapy License
Education Requirements: None specified
Years of Experience: Minimum of 1 year of relevant experience
Location Requirements: Must have current Driver's License & Auto Insurance
Schedule: Monday through Friday, 8:00 AM 5:00 PM, with a minimum of 40 hours per week
Responsibilities: Conduct start of care, recertification, and resumption of care visits (approximately 6.5 points per day). May perform assessments, educate patients, and provide support related to home health therapy services.
Additional Information: No clinical involvement outside of manager interview. Travel is within a 50-mile radius; mileage will not be paid. Minimal orientation with a short-term preceptor. No weekends or holidays required.
