Physical Therapist (PT) specializing in Home Health (HH) services in the Sacramento area. This role involves providing outpatient therapy in patients' homes, including assisted livings, senior apartments, and care homes. The position requires delivering comprehensive care to a diverse patient population across various diagnoses, with a flexible work environment supporting efficient and patient-centered care.
Job Details:
- Schedule: M-F, 8-hour shifts from 8:30 am to 5:00 pm, with a minimum of 40 hours per week.
- Required Qualifications: Certifications in Basic Life Support (BLS) from the American Heart Association (AHA); valid California Physical Therapist license; minimum 2 years of experience in a healthcare setting, with community/home health experience preferred; current driver's license and proof of automobile insurance.
- Preferred Qualifications: Homecare Home Base (HCHB) electronic medical record experience; prior home health experience including OASIS.
- Additional Information:
- Candidates must have a clean legal name on government-issued ID; no employment with affiliated healthcare organizations within the past year.
- Home Health services include treatments in patients' homes, assisted livings, and senior living communities.
- Caseload typically includes 3 to 5 patients daily, with two Start of Care visits and evaluations.
- Supervision of up to two PTA/COTAs supervised by an RN Manager supported by a Regional Rehab Leader/Educator.
- Responsibilities include managing a variety of diagnoses such as orthopedic, cardiovascular, pulmonary, oncology, neurological, geriatrics, and amputations.
- The position entails working in sometimes cramped or cluttered environments and adhering to a business casual dress code with required safety attire.
- Flexibility with holiday work and one weekend per month is requested but not mandatory.
- Orientation includes a three-week online and in-field precepting process, with solo work expected by end of week two or three.
- Mileage reimbursement is at the GSA rate with prior approval.
- Cancellation policy allows up to four shifts canceled within a 13-week assignment.
- Charting uses Homecare Home Base (HCHB).
