Job Details:

  • Schedule: M-F, 8-hour shifts from 8:30 am to 5:00 pm, with a minimum of 40 hours per week.
  • Required Qualifications: Certifications in Basic Life Support (BLS) from the American Heart Association (AHA); valid California Physical Therapist license; minimum 2 years of experience in a healthcare setting, with community/home health experience preferred; current driver's license and proof of automobile insurance.
  • Preferred Qualifications: Homecare Home Base (HCHB) electronic medical record experience; prior home health experience including OASIS.
  • Additional Information:
    • Candidates must have a clean legal name on government-issued ID; no employment with affiliated healthcare organizations within the past year.
    • Home Health services include treatments in patients' homes, assisted livings, and senior living communities.
    • Caseload typically includes 3 to 5 patients daily, with two Start of Care visits and evaluations.
    • Supervision of up to two PTA/COTAs supervised by an RN Manager supported by a Regional Rehab Leader/Educator.
    • Responsibilities include managing a variety of diagnoses such as orthopedic, cardiovascular, pulmonary, oncology, neurological, geriatrics, and amputations.
    • The position entails working in sometimes cramped or cluttered environments and adhering to a business casual dress code with required safety attire.
    • Flexibility with holiday work and one weekend per month is requested but not mandatory.
    • Orientation includes a three-week online and in-field precepting process, with solo work expected by end of week two or three.
    • Mileage reimbursement is at the GSA rate with prior approval.
    • Cancellation policy allows up to four shifts canceled within a 13-week assignment.
    • Charting uses Homecare Home Base (HCHB).
