Job Summary
Physical Therapist Home Health (PT HH) in Sacramento offers an opportunity to provide patient-centered care within a home health setting. This role involves delivering therapy services to patients in their homes, including assessments, treatments, and evaluations, primarily serving the Mercy San Juan Hospital area and surrounding communities.
Job Details
- Position Description: Provide physical therapy to patients in various home environments, including assisted livings and senior apartments, with varied diagnoses such as orthopedic, respiratory, neurological, and geriatric conditions.
- Certifications, Education, and Experience Requirements: Valid Physical Therapist license in California, BLS (AHA) certification, current driver's license, proof of automobile insurance, with at least 2 years of experience in a healthcare setting. Home health experience including OASIS is preferred.
- Schedule: Monday through Friday, 8:30 AM to 5:00 PM, with a minimum of 40 hours per week.
- Additional Responsibilities: Engage in a robust 3-week orientation process, including online learning and in-field precepting. Solo work expected by the end of the second week.
- Submission Requirements: Candidates must submit a copy of their drivers license and appropriate certifications.
- Additional Information: Must work in home environments that may be cramped or cluttered; travel is required across designated territories to serve patients from multiple hospitals and community providers. Mileage will be reimbursed at the GSA rate with prior approval.
