Job Summary:
Physical Therapist (PT) specializing in Home Health located in Lexington, Kentucky. This role entails providing outpatient therapy services within a home health setting, with a focus on patients requiring care for conditions such as CHF, orthopedic issues, and pneumonia. The position involves performing start of care, recertifications, evaluations, and daily visits, with a scheduled shift of eight hours per day.
Job Details:
- Shift Schedule: 8:00 AM to 5:00 PM, Monday through Friday, with rotating weekends and holiday rotation.
- Required Qualifications: Valid license to practice as a Physical Therapist in the state of Kentucky, BLS (American Heart Association) certification, and a minimum of 1 year of relevant clinical experience.
- Preferred Qualifications: No requirement for new graduates; candidates with home health and home care experience are preferred.
- Additional Information: No formal orientation required; two-week preceptorship with a work coach. A clinical manager or educator will be available 24/7 for emergencies or during regular business hours. Scrubs (color) or business casual attire are acceptable. A phone and tablet will be provided for clinical use. The employer permits cancellation up to four shifts in a 13-week assignment, or three shifts for shorter assignments. Charting will be conducted using Homecare Homebase (HCHB).
