Key Responsibilities

• Provide physical therapy services to patients in a home health setting across assigned coverage areas including Mansfield, Midlothian, Waxahachie, Arlington, and Northeastern Fort Worth
• Conduct patient evaluations and develop individualized treatment plans to address functional goals
• Deliver approximately 35–40 patient visits per week
• Document patient progress, treatment plans, and clinical notes in accordance with regulatory and organizational standards
• Collaborate with interdisciplinary care teams to coordinate patient care and ensure continuity of services

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas (or eligibility to obtain)
• Graduate of an accredited Physical Therapy program
• Valid driver's license and reliable transportation for home health visits across the coverage territory
• Ability to manage a caseload of approximately 35–40 visits per week

Preferred Experience / Skills

• Prior experience in a home health setting is a plus; new graduates are welcome to apply
• Strong time management and organizational skills to support an independent, field-based caseload
• Familiarity with electronic medical records (EMR) documentation
