Job Summary
Physical Therapist (Home Health) in the area is responsible for providing patient-centered therapy services within a designated territory. The role involves delivering high-quality care to geriatric patients through home visits, ensuring timely documentation, and adhering to clinical protocols to optimize patient outcomes.
Job Details
- Certifications Required: BLS (Basic Life Support)
- Education Requirements: Active license in hand
- Location Requirements: Must have an active drivers license and personal transportation; reliability for flexible territory coverage preferred
- Years of Experience Required: Minimum of 1 year relevant clinical experience in home health or similar setting, including OASIS and proficiency in HCHB (Homecare Homebase)
- Schedule: Days, 8-hour shifts, Minimum of 40 hours per week
- Additional Job Information:
- Travel within up to a 60-mile radius from the hub/office, occasionally more based on patient needs
- All travelers are required to have an active drivers license, and personal transportation
- RTO (Requested Time Off) must be submitted at the time of application
- On-call rotation may be requireddiscuss during interview
- Timely communication and documentation are mandatory
- All travelers are required to attend a Monday orientation
- Modules are billable and completed during orientation
- Former permanent employees must be separated for a minimum of 6 months before returning as travelers and require HR approval
- Travel Pairs accepted with individual territories and transportation required
