Job Details
- Specialty: Physical Therapist (Home Health)
- Location: Outpatient Clinic at a critical access facility with 25 beds and 9 visits per day.
- Schedule: Days shift, 8 hours per day, minimum of 40 hours weekly.
- Certifications: BLS and valid state license required.
- Required Qualifications: PTA license, active or pending Illinois license, minimum 1 year of outpatient clinic experience, experience taking vital signs, and familiarity with assessment, discharge planning, short-term goal setting, and time management skills.
- Experience: 1 year of relevant outpatient therapy experience preferred.
- Additional Information: No float required; candidate will not float within scope. Must have outpatient rehab experience, including assessments and patient education.
- Candidate must review and understand role requirements, complete compliance documentation, and confirm housing arrangements prior to start.
