Responsibilities

• Complete Start of Care (SOC), evaluations, reassessments, and discharges
• Develop and manage patient plans of care
• Supervise PTA staff
• Manage and schedule your own patient caseload
• Provide home health therapy services throughout York and Eastern Adams County

Requirements

• Active Physical Therapist license
• Graduate of an accredited PT program
• CPR (if required)
• 1+ year of Home Health experience highly preferred
• Home Care Home Base (HCHB) experience highly preferred
• OASIS experience preferred
