Position Details

• Contract Duration: 13 Weeks

• Schedule: Monday-Friday, 8:00 AM-5:00 PM

• Hours: 40 Hours per Week

• On-Call: On-call required if requested. On-call needs are subject to change based on staff and branch needs.

• Productivity Units: 30 Units

• Qualifications: HCHB, OASIS, and Home Health experience required.

Required Skills & Experience

• Current State License as a Physical Therapist - Required.

• Proof of current CPR certification - Required.

• Current driver's license, reliable transportation, and insurance - Required.

• 1-2 years of experience as a Physical Therapist in an acute care or rehabilitation setting - Required.

• Previous home care experience - Preferred.

• HCHB EMR experience - Preferred.

Required Education

- Bachelor of Science in Physical Therapy from an American Physical Therapy Association (APTA)-approved program - Required.
