Job Title: Physical Therapist (PT) - Home Health | Pueblo, CO (Local or Travel)

Location: Pueblo, CO
Employment Type: Contract (13 Weeks)
Schedule: Monday - Friday, 8:00 AM - 5:00 PM (40 hours/week)
Targeted to cover Pueblo and east
30 units
32 hours guaranteed
HH required

Position Overview

We are seeking an experienced Physical Therapist (PT) for a Home Health assignment based in Pueblo, CO. This position involves travel across multiple counties and offers consistent weekday hours with guaranteed weekly hours.

Qualifications

• Experience:

  • 1-2 years as a Physical Therapist in an acute care or rehabilitation setting - Required

  • Previous Home Health experience - Preferred

  • HCHB (Homecare Homebase) EMR experience - Preferred

• Licenses & Certifications:

  • Current State License as a Physical Therapist - Required

  • Current CPR certification - Required

  • Valid driver's license with reliable transportation and insurance - Required

• Education:

  • Bachelor of Science in Physical Therapy from an APTA-approved program - Required
