We're seeking a compassionate and motivated Physical Therapist for a 13-week travel assignment in Grand Island, Nebraska. This is a unique opportunity to work across both Skilled Nursing Facility (SNF) and Home Health settings while enjoying schedule flexibility and the support of an experienced therapy leadership team.

Assignment Details

Contract Length: 13 Weeks
Location: Grand Island, NE
Setting: Post-Acute Care (SNF & Home Health)
Guaranteed Hours: 40 hours per week
Schedule Options:
- Monday–Friday
- Tuesday–Saturday
- Sunday–Thursday
Shift Length: 8-hour days (daily hours may vary slightly)

Position Overview

This position provides a balanced caseload, spending approximately 50% of the time in Skilled Nursing Facilities and 50% in Home Health. The primary coverage area is Grand Island, with occasional travel to Hastings or Kearney to assist with PTO coverage or ensure full-time hours.

The Physical Therapist will evaluate and treat patients, develop individualized care plans, and collaborate with an interdisciplinary team to help patients achieve their rehabilitation goals.

Responsibilities

Evaluate and treat patients in both SNF and Home Health settings
Develop individualized treatment plans and document patient progress
Supervise up to two Physical Therapist Assistants (PTAs)
Collaborate with physicians, nursing staff, and the rehabilitation team
Provide patient and caregiver education
Maintain accurate documentation using electronic medical records

Qualifications

Current Nebraska Physical Therapist license
Minimum 1 year of Physical Therapy experience
Current AHA BLS Certification
Graduate of an accredited Physical Therapy program
Strong communication, organizational, and patient care skills
