Key Responsibilities

• Conduct physical therapy evaluations and provide subsequent visits for home health patients within an approximately 40-mile coverage territory
• Expected caseload of 2 evaluations and 3–4 subsequent visits, or 5–6 total visits per day
• Oversee and supervise the work of Physical Therapist Assistants (PTAs) in the field
• Document all patient care and clinical notes using the HCHB EMR system
• Complete a minimum of 2 days of in-office orientation during the first 2 weeks of the assignment

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Experience in a home health setting
• Ability to travel within an approximately 40-mile service radius
• Proficiency with or ability to learn the HCHB electronic medical records system
• Valid driver's license and reliable transportation required for field-based visits

Preferred Experience / Skills

• Prior experience supervising or collaborating with Physical Therapist Assistants (PTAs)
• Familiarity with home health documentation standards and productivity expectations
• Strong time management skills to effectively manage a daily visit caseload
