Job Summary:
The Physical Therapist Home Health (PT HH) position is located in Minneapolis, Minnesota. This role involves assessing clients' needs, developing personalized therapy plans, and performing therapeutic interventions in a home health setting. The position offers the opportunity to provide direct patient care while collaborating with interdisciplinary teams to support patient independence and recovery.

Job Details:

  • Schedule: 8-hour days; open to 4-10s; minimum of 40 hours per week
  • Required Qualifications: Completion of an accredited physical therapy program; current BLS (Basic Life Support) certification; state licensure and registration if applicable; minimum 1 year of clinical experience; ability to communicate effectively in English
  • Preferred Qualifications: Prior experience in a home health setting is helpful, but not required
  • Float: Float rotation may be required
  • Additional Information: This position involves implementing therapy plans, maintaining client confidentiality, notifying physicians of changes, and documenting services provided. It requires supervising therapy assistants in accordance with federal and state regulations, following infection control standards, and using appropriate precautions.
