Amergis Healthcare Staffing, Inc.-Allied is seeking a travel Physical Therapist for a travel job in Hazle Township, Pennsylvania.
This Home Health Physical Therapist will provide skilled therapy services to patients in their homes throughout Schuylkill County, including evaluations, start of care visits, follow-ups, recertifications, transfers, and discharges. The role requires documentation using Home Care Home Base (HCHB) and OASIS, managing an average of six visits per day, and collaborating with the care team to ensure quality patient outcomes. This is a 13-week contract starting 11/16/2026, working Monday through Friday from 8:00 AM to 4:30 PM, with no weekends or on-call requirements. Home health experience and HCHB experience are strongly preferred.
