Schedule

• Monday - Friday

• 8:00 AM - 5:00 PM

• 40 hours per week

• Guaranteed Work Week: 40 hours

Pay

• Pay Rate + IRS Mileage Reimbursement: $0.76/mile

Coverage Area

Sapphire and Ruby, including:

• Sacramento

• Natomas

• Woodland

• Davis

• Lemon Hill

• Freeport

May be asked to assist in other territories within the facility's service area as needed, which is approximately a 60-mile radius from the facility address.

Caseload & Productivity

• Average 4-6 visits per day, depending on the type of visit for PT/OT/SLP.

• Productivity requirement: 28-32 points per week for PT/OT/SLP.

Productivity Points:

• SOC: 2.5 points

• ROC: 2 points

• Evaluations/Recertification/Discharge OASIS: 1.5 points

• Discipline DC, Routine Visit, 30-Day Reassessment: 1 point

• Weekly Case Conference: 1 point

Requirements

• Home Health experience required - no exceptions.

• OASIS experience required - no exceptions.

• Homecare Homebase (HCHB) documentation experience required.

Additional Information

• 2-week cancellation policy.

• Any overtime must be pre-approved and requires written approval from the Administrator.
