Schedule / Job Type

Contract (13 Weeks) | Full-Time | Days | Monday – Friday with one weekend day (Saturday or Sunday) rotation every 4 weeks

Key Responsibilities

  • Provide physical therapy evaluations and treatments to patients within an assigned home health territory
  • Complete OASIS assessments in accordance with home health requirements
  • Document patient care and progress using HCHB (HomeCare HomeBase) EMR
  • Meet a productivity expectation of 28 points per week
  • Oversee and supervise the work of Physical Therapist Assistants (PTAs)
  • Travel to patient homes within an approximately 40-mile radius of the office; schedule flexibility available during inclement weather

Required Qualifications

  • Active Physical Therapist (PT) license in the state of Connecticut
  • Prior home health experience required
  • OASIS experience required
  • Ability to travel throughout the assigned territory (Hartford, West Hartford, Bloomfield, Avon, Simsbury, Granby, Suffield, Windsor, Windsor Locks, East Hartford, Wethersfield, and Rocky Hill)

Preferred Experience / Skills

  • Experience with HCHB (HomeCare HomeBase) EMR system
  • Experience supervising or working alongside PTAs
  • Comfort with independent scheduling and self-directed patient management in a home-based setting
