Key Responsibilities

• Evaluate and treat patients in an outpatient physical therapy setting
• Develop and implement individualized treatment plans based on patient needs and clinical findings
• Document patient progress and maintain accurate clinical records in accordance with facility requirements
• Collaborate with the interdisciplinary care team to ensure optimal patient outcomes
• Adhere to all credentialing, compliance, and documentation requirements as outlined by the facility

Required Qualifications

• Active Physical Therapist license required; must obtain permanent Arizona state licensure promptly upon assignment start
• Prior experience in an outpatient physical therapy setting
• Ability to meet facility-specific credentialing and quality assurance requirements
• Compliance with all facility onboarding and documentation processes prior to start date

Preferred Experience / Skills

• Previous experience working in a high-volume outpatient therapy environment
• Familiarity with outpatient documentation systems and clinical workflows
