Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Evaluate, develop, and implement individualized patient care plans
• Supervise and oversee Physical Therapist Assistants (PTAs)
• Document patient progress and clinical notes using HCHB EMR
• Participate in a 2-day in-office orientation at the start of the assignment

Required Qualifications

• Active Physical Therapist (PT) license in the state of Texas
• Experience in a home health setting
• Must reside within 40 miles of the facility to be eligible
• No on-call or weekend availability required

Preferred Experience / Skills

• Prior experience with HCHB electronic medical records system
• Experience supervising or mentoring PTAs
• Strong time management and independent practice skills in a field-based environment
