Job Summary:
Physical Therapist (PT) with specialization in Home Health located in Albemarle, North Carolina. This position involves providing patient-centered care for adult patients with chronic diseases, orthopedic issues, and other conditions within a home health setting. The role requires travel throughout designated areas, managing patient plans of care, and maintaining compliance with home health standards.

Job Details:

• Schedule: Days shift, 8 hours per day, minimum of 40 hours per week, start date 08/24/2026.
• Required Qualifications: State licensure, valid drivers license with proof of insurance, BLS certification, minimum of 1 year of Home Health experience, proficiency with Homecare Home Base electronic medical record system, experience with geriatric and pediatric patients, chronic disease management, and OASIS-E proficiency.
• Preferred Qualifications: Experience with gait training, balance training, non-pharmaceutical pain management, and creating individualized plans of care to optimize patient outcomes.
• Support within the Department: Departmental leadership, schedulers, clinical managers, clinical supervisors, and peer clinical team.
• Technology/Equipment: EPIC Rover (view-only), Focura (discharge instructions and wound documentation), Samsung Note 9 phones, Home Care Home Base EMR system.
• Orientation (facility and unit): New Employee Orientation (NEO), 5-day orientation program.
• Scheduling: No weekend rotation, no on-call, no major holidays. Visits are scheduled by the clinician, with patients scheduled the day prior to visits.
• Additional Notes: Scrub color: gray; parking: free; team-oriented department culture.
• Other Information: No company car provided; company cell phone supplied for documentation and communication. Visits are scheduled by the therapist with no scheduled weekend work; cancelled appointments do not count as visit equivalents; therapists are required to complete 30 visit equivalents per week.
