Job Description:

This position involves providing physical therapy services in a home health setting. The role requires evaluating and treating patients with a variety of diagnoses including CHF, orthopedic conditions, and pneumonia. The physical therapist will work in multiple zip code areas and must be able to travel between them.

Responsibilities:

• Evaluate and treat patients in a home health setting
• Complete SOC, ROC, Recert, Evals, and regular visits
• Communicate effectively with the clinical manager or educator
• Utilize a computerized charting system

Requirements:

• 1 year of experience in home health physical therapy
• BLS certification required
• Preferred candidates should have experience treating common diagnoses such as CHF, orthopedic conditions, and pneumonia
