Key Responsibilities

  • Conduct comprehensive patient evaluations (60-minute evals) and deliver follow-up treatments (30-minute sessions), developing and modifying individualized treatment plans in accordance with patient needs, goals, and physician direction
  • Complete medical record documentation in a clear, concise, accurate, and timely manner, including goal writing, functional quality indicator assessments, letters of medical necessity, and charge entry in compliance with state, federal, and intermediary guidelines
  • Design functional treatment programs to support effective patient return to community activities, progressing patients toward discharge within an established length of stay
  • Delegate and direct discipline-specific therapy services for assigned patients while ensuring care is provided under signed, valid physician orders and within insurance approvals
  • Participate in student programs as assigned, providing appropriate supervision, developing learning objectives, and completing student evaluations
  • Adhere to discipline-specific State Practice Act and all regulatory agency requirements, maintaining professional licensure in good standing

Required Qualifications

  • Active Physical Therapist license in good standing in the state of Colorado
  • Ability to complete comprehensive patient evaluations and develop evidence-based treatment plans
  • Proficiency in clinical documentation, goal writing, and compliance with state and federal guidelines
  • Commitment to patient and employee safety standards and safe patient handling practices

Preferred Experience / Skills

  • Prior experience in an outpatient physical therapy setting
  • Experience supervising or mentoring PT students
  • Active involvement in continuing education and professional development activities
