Job Summary
Outpatient Physical Therapist (PT OP) The organization is seeking a dedicated outpatient Physical Therapist to evaluate, develop, and implement treatment plans for patients in an outpatient setting. This role involves coordinating patient care, ensuring safety, and providing education to patients and families to facilitate a successful return to community activities.

Job Details

  • Position Description: Evaluates all assigned patients and develops, implements, and modifies treatment plans according to patient needs, goals, and physician directives. Responsible for delegation and supervision of therapy services for assigned patients and ensuring functional treatment design. Serves as an educator for patients, families, students, and coworkers.
  • Certifications, Education, and Experience: Degree in Physical Therapy from an accredited school, CPR Certification, active license for the state, and at least 1 year of outpatient clinical experience.
  • Schedule: Days shift, 8 hours per day, minimum of 40 hours per week.
  • Responsibilities: Completes comprehensive evaluations, utilizes clinical reasoning to develop appropriate treatment plans, documents medical records accurately and timely, ensures care is provided under signed physician orders, and participates in student programs and professional development activities.
