Key Responsibilities

• Provide physical therapy evaluation and treatment to patients in acute care and rehabilitation settings
• May also support outpatient therapy services as needed
• Develop and implement individualized treatment plans in accordance with physician orders and clinical best practices
• Document patient progress, treatment plans, and outcomes in compliance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to optimize patient outcomes and transitions of care

Required Qualifications

• Active Georgia Physical Therapist (PT) license required
• BLS certification required
• Minimum 1 year of physical therapy experience required
• Experience in acute care and/or rehabilitation settings required

Preferred Experience / Skills

• Outpatient physical therapy experience
• Familiarity with hospital-based interdisciplinary care models
