The PT develops or contributes to the developmentof the home health plan of care under the direction of the physician andfacilitates appropriate interdisciplinary involvement to ensure the bestoutcome for each patient. The PT delivers therapy intervention and treatmentdirected by the physician as established in the home health plan of care thatis consistent with clinical best practices and results in high quality,improved outcomes and exceptional patient experience.ResponsibilitiesPatientCare/Care Coordination Makes visitsto the homes of patients requiring home health physical therapy services andassumes responsibility for an ongoing interdisciplinary assessment of thepatient. Performsinitial and comprehensive assessments on all patients in accordance with agencypolicies and procedures. Evaluates andassesses the patient s level of functioning by applying appropriate tests. Treats clientsto relieve pain, develop and restore function and maintain maximum performanceusing appropriate physical therapy treatments, modalities and procedures asstated in the physical therapy plan of care. Initiatesdiagnostic, preventative and rehabilitative procedures as appropriate to thepatient s care delivery Ensuresquality and safe delivery of services to patients according to the care plan. Uses equipmentand supplies safely, effectively, and efficiently. Promotespatient independence by teaching patients and family members to understand thefollowing as well as any additional education needs identified by the assessingclinician and/or physician:oTreatment anddisease managementoMedicationspurpose, use, side effects, potential adverse effectsoProper use,safety hazards, and infection control issues related to the use and maintenanceof any equipment providedoPatient planof careoEmergencypreparednessoPhases ofphysical therapy in which they may assist and use of prosthetic and orthoticdevices and medical and adaptive equipment Provides theservices that are ordered by the physician as indicated in the plan of care. Providespatient, caregiver and family counseling and education as indicated in the planof care. Develops andcontinually updates a plan of care that specifies the care and servicesrequired to meet patient-specific needs determined during the comprehensiveassessment and includes measureable outcomes that will occur as a result ofimplementing and coordinating the plan of care. Develops andupdates patient s discharge plan on every visit ensuring patient and caregivereducation and training is facilitating timely discharge and measureable goalsand outcomes are attainable, realistic and up-to-date. Includes thepatient, representative and caregiver in all plan of care decisions. Assesses theneed for plan of care updates and every visit and includes the patient andfamily in care planning decisions and communicates care plan revisions to thepatient, representative, caregiver and all physicians issuing orders for thehome health plan of care. Maintainsaccurate and timely documentation of clinical records and time/travel entries. Receives,relays and documents verbal orders in accordance with state and federalregulations and in a timely manner to prevent adverse patient outcomes. Is responsiblefor obtaining physician orders prior to initiation of care and notifying thephysician of any changes in the patient s condition. Communicateswith all physicians involved in the plan of care and other health carepractitioners related to the current plan of care. Identifiespatient s risks for hospitalization and/or emergency department use andproactively intervenes to prevent adverse events. Superviseshome health aide and physical therapy assistant staff and documents supervisionin the patient medical record.Home CarePerformance Criteria Deliversquality care and completes clear, concise and accurate clinical documentationof care provision in accordance with home health conditions of participation(CoPs), various payer home health coverage criteria, accreditation standards,professional standards for discipline/credential and the home health plan ofcare. Maintainscompetence in OASIS data collection and documentation and submits all initialand corrected assessments according to CMS guidance and agency policies andprocedures. Participatesin clinical review of records to monitor quality and accuracy, appropriateutilization, appropriate involvement of interdisciplinary team members,appropriate referrals and coordination of care. Passed an examination for Physical Therapists approved by the state in which physical therapy services are provided. Licensed as a PTby the state in which practicing, unless licensure does not applywithin state regulation One yearexperience caring for similar patient population as to be assigned. Basic knowledge of current, frequently used medications, including modes of administration, actions and side effects. Able to perform physical therapy assessment and treatment in the home setting without direct supervision. Strong interpersonal skills. Ability to work as a collaborative team member. Ability to understand and apply guidelines, policies and procedures. Ability to navigate and perform basic use of Microsoft Office products. Maintains a reliable mode of transportation andhas the ability todrive safely during all day and night hours and in all types of inclement weather.
