Job Summary
Home Healthcare Nurse This role is based in a home health setting, providing patient care with a focus on patients diagnosed with conditions such as CHF, ORTHO, and PNEUMONIA. The position requires the nurse to work primarily in a community-based environment, delivering comprehensive patient assessments, treatments, and patient education. The assignment is structured for immediate start and expected to run from September 28, 2026, to December 28, 2026.
Job Details
- Position Description: No clinical involvement; profiles are submitted directly to the manager for consideration. Must have a valid license in hand at the time of submission.
- Experience Requirements: Must have a valid state license. Minimum of 1 year of home health experience and Home Care Home Base experience.
- Schedule: Monday through Friday, 8:00 AM to 5:00 PM, with occasional weekend rotation and holiday coverage, averaging 40 hours per week. The weekend shift hours are 8:00 AM to 5:00 PM on a rotating schedule.
- Certifications: BLS (American Heart Association) required.
- Responsibilities: Conducts start of care, recertification, reassessments, evaluations, and visits for home health patients. Ensures compliance with clinical standards and documentation procedures using Home Care Home Base.
- Additional Information: Phone and tablet will be provided for clinical documentation and communication. A two-week orientation with a preceptor is included, with some visits with a work coach.
