Key Responsibilities

• Review, validate, and accurately dispense medication, supply, and equipment orders for inpatient and/or outpatient populations in compliance with federal and state law and facility policies
• Monitor and assess patient medication and nutritional therapies, documenting clinical interventions in a timely manner and optimizing pharmacotherapy in accordance with approved protocols
• Consult with physicians and interdisciplinary healthcare providers to ensure safe, effective, and cost-efficient medication therapy management
• Guide, direct, and train pharmacy technicians, interns, and new pharmacist personnel; assume responsibility for the quality and quantity of their work
• Participate in interprofessional patient care rounds, transition of care processes, and medication reconciliation activities as assigned
• Respond to emergency alerts (e.g., Code Blue, Code Sepsis, Code Stroke) as a top priority when assigned to hospital settings

Required Qualifications

• Active California Registered Pharmacist license (must be in hand)
• Doctor of Pharmacy (Pharm.D.) degree required
• Minimum of 1 year of clinical pharmacist experience required
• Inpatient candidates: completed PGY-1 residency or at least 2 years of clinical hospital pharmacist experience
• Outpatient candidates: completed PGY-1 residency or at least 2 years of clinical pharmacist experience
• Must live more than 150 miles from the facility to be eligible; local candidates are not accepted

Preferred Experience / Skills

• Completion of a PGY-1 or PGY-2 pharmacy residency program in a relevant specialty area (e.g., Emergency Medicine, Critical Care, Infectious Disease, Oncology, Ambulatory Care)
• Board certification relevant to area of specialty (e.g., BCPS, BCACP, BCOP, CGP, or Certified Diabetes Educator)
• Prior experience in a specialty pharmacy area such as Emergency Medicine, Oncology, Infectious Disease, Critical Care, Home Infusion, or Ambulatory Care
• Experience serving as a preceptor for pharmacy residents, interns, or students
