Key Responsibilities

• Review, interpret, and dispense medications according to professional standards and state and federal laws and regulations across outpatient, inpatient, and skilled nursing facility settings
• Perform clinical interventions including therapeutic drug monitoring, renal dosing adjustments, and medication profile reviews to prevent adverse drug reactions, therapeutic duplications, and drug interactions
• Conduct monthly medication regimen reviews (MRR) for skilled nursing facility patients and participate in quarterly Continuous Quality Improvement (CQI) meetings
• Counsel patients on medication therapies and provide drug information to physicians, nursing staff, and other healthcare team members
• Verify and prepare medications by compounding, packing, and labeling; oversee and organize ancillary staff workflow including pharmacy technicians and clerks
• Maintain compliance with California Board of Pharmacy, DEA, and FDA regulations and uphold a safe, clean working environment

Required Qualifications

• Doctorate of Pharmacy (PharmD) degree required
• Current, active California Pharmacy License
• Minimum of one year of pharmacy experience
• Proficiency with Automated Dispensing Units (e.g., PYXIS), IV Laminar Flow Hood, and electronic health record (eHR) systems
• Strong verbal and written communication skills in English
• Ability to prioritize workload, work independently, and supervise support personnel

Preferred Experience / Skills

• Experience working in outpatient, inpatient, and/or skilled nursing facility pharmacy environments
• Familiarity with CPOE systems and medication error reporting platforms
• Proficiency in additional languages beyond English
