Key Responsibilities
Dispense and verify outpatient prescriptions accurately and in compliance with applicable regulations
Perform drug utilization reviews and monitor for drug interactions, contraindications, and dosing appropriateness
Collaborate with clinical staff and providers to optimize patient medication therapy
Counsel patients on proper medication use, side effects, and adherence
Maintain accurate patient medication records and documentation
Required Qualifications
Doctor of Pharmacy (Pharm.D.) degree or equivalent from an accredited institution
Active and unrestricted Pharmacist license in the state of New York
Experience in an outpatient or retail pharmacy setting
Strong knowledge of pharmaceutical regulations and dispensing procedures
Preferred Experience / Skills
Experience working within a health system or hospital-affiliated outpatient pharmacy
Familiarity with electronic health record (EHR) and pharmacy management systems
Strong patient communication and counseling skills
