    Job Title

    Pharmacist – Ambulatory Care (Clinical)

    Location

    Arlington, TX | Outpatient / Primary Care

    Schedule / Job Type

    Contract | Onsite | Days | Weekdays Only | 4x10-hour shifts (7:00 AM – 7:00 PM window)

    Key Responsibilities
    • See approximately 16 patients per day as a mid-level provider across two primary care clinic locations
    • Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements
    • Perform drug utilization reviews, monitor for drug interactions and potential adverse events, and work to minimize polypharmacy
    • Develop patient medication action plans with self-management goals and communicate clinical interventions as part of the multidisciplinary care team
    • Serve as preceptor for pharmacy students or residents and contribute to didactic and experiential education programs
    • Assist in the development of Pharmacy and Therapeutics (P&T) policies, formulary analysis, and pharmacy safety initiatives including Joint Commission Standards compliance
    Required Qualifications
    • BCACP (Board Certified Ambulatory Care Pharmacist) certification required
    • PGY2 Ambulatory Care Pharmacy Residency required
    • Active Pharmacy license from the Texas State Board of Pharmacy required
    • BLS certification required
    • Minimum 4 years of direct patient care experience demonstrating medication management
    • EPIC electronic health record experience required
    Preferred Experience / Skills
    • Experience working within a Medical Home or multidisciplinary primary care team model
    • Experience providing drug information, population management support, and in-service education to medical and nursing staff
    • Familiarity with Pharmacy and Therapeutics committee processes and formulary management
