General Summary:
The Population Health Pharmacist is a licensed pharmacist providing clinical pharmacy support to Medicare and Commercial providers within value-based care arrangements at facilty. Responsibilities focus on improving quality of pharmacy-related care (STAR measures) and cost containment strategies for Delaware residents.
Responsibilities:
Analyze reports and EMR data to identify opportunities for improving outcomes (STAR/HEDIS) and reducing pharmacy costs.
Assess health plan members with pre-determined conditions, identify barriers to care, provide self-management education, recommend screenings, perform MTM services, and develop personalized health improvement plans.
Collaborate with providers on clinical quality data initiatives; maintain a 5-STAR rating of 90% or greater annually.
Participate in committees, meetings, and audits; develop and maintain policies and procedures.
Educate providers and members on formulary changes annually.
Identify member needs and intervene to prevent avoidable hospital admissions/readmissions; document interventions.
Act as subject matter expert in pharmacy-related clinical measures for value-based care agreements.
