License: Pharmacy license or eligibility for the same. Certifications: BLS Must-Have: Doctorate of Pharmacy or equivalent from an accredited College of Pharmacy, 2-year residency in HIV. Description: Provide clinical leadership for pharmaceutical care in area(s) of responsibility. Serve as a resource for outpatient and inpatient clinicians for HIV and/or Hepatitis pharmacotherapy. Work with the Inpatient Infectious Diseases Clinical Pharmacist to transition HIV patients from the hospital setting to the HIV Clinic on appropriate therapy. Develop and optimize pharmacotherapeutic regimens through collaborative practice agreements. Perform drug utilization reviews and monitor for drug interactions and potential adverse events. Work to minimize polypharmacy in patients and develop patient medication action plans with self-management goals. Act as a preceptor for pharmacy students or residents and participate in didactic and experiential education. Contribute as a key component of the core multidisciplinary team involved in patient care.
