Responsibilities
  • Review, verify, and dispense medications accurately in accordance with physician orders and clinical guidelines
  • Evaluate medication therapies for appropriateness, effectiveness, drug interactions, allergies, and contraindications
  • Prepare and compound medications, including sterile preparations, when required
  • Monitor patient medication regimens and recommend therapeutic alternatives as appropriate
  • Provide medication counseling and education to patients, families, nurses, and healthcare providers
  • Collaborate with physicians, nurses, and interdisciplinary healthcare teams to optimize medication therapy and patient outcomes
  • Participate in antimicrobial stewardship, medication safety initiatives, and quality improvement programs
  • Ensure compliance with state and federal pharmacy regulations, Joint Commission standards, and hospital policies
  • Maintain accurate documentation and medication records using pharmacy information systems
  • Supervise and support pharmacy technicians while ensuring efficient pharmacy operations
  • Respond promptly to medication-related emergencies and after-hours pharmacy needs
Requirements
  • Active Texas Pharmacist License
  • Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy from an accredited program
  • Minimum 2 years of recent hospital or acute care pharmacist experience
  • Current BLS Certification preferred
  • Experience with computerized physician order entry (CPOE), pharmacy information systems, and electronic medical records (EMR)
  • Knowledge of sterile compounding and IV admixture preferred
  • Strong understanding of pharmacotherapy, medication safety, and clinical pharmacy practices
  • Excellent communication, analytical, and problem-solving skills
  • Ability to work independently during night shifts while collaborating effectively with healthcare teams
  • Commitment to delivering safe, accurate, and patient-centered pharmaceutical care
