This role works collaboratively with physicians, nurses, pharmacy technicians, and other members of the interdisciplinary care team to optimize medication therapy and promote positive patient outcomes. Key responsibilities include reviewing and verifying medication orders for accuracy, appropriate dosing, allergies, interactions, contraindications, and therapeutic effectiveness. Dispensing medications in accordance with physician orders, hospital policies, and state and federal regulations. Providing clinical recommendations regarding medication selection, dosing, monitoring, and adjustments. Monitoring patient medication therapies, laboratory results, renal/hepatic function, and other relevant clinical information. Participating in medication reconciliation and supporting safe transitions of care. Preparing, verifying, and overseeing sterile and non-sterile medication compounding as applicable.
