Job Title: Pharmacist Profession: Pharmacy Specialty: Pharmacist Duration: 13 weeks Shift: 12:00 AM - 12:00 AM Hours per Shift: 12 Experience: Minimum 2 years License: Active pharmacist license Certifications: N/A Must-Have: Experience with inpatient pharmacy operations Description: The Pharmacist is responsible for providing safe, accurate, and effective medication management for patients within the hospital setting. This role works collaboratively with physicians, nurses, pharmacy technicians, and other members of the interdisciplinary care team to optimize medication therapy and promote positive patient outcomes. Key responsibilities include reviewing and verifying medication orders for accuracy, appropriate dosing, allergies, interactions, contraindications, and therapeutic effectiveness. Dispensing medications in accordance with physician orders, hospital policies, and state and federal regulations. Providing clinical recommendations regarding medication selection, dosing, monitoring, and adjustments. Monitoring patient medication therapies, laboratory results, renal/hepatic function, and other relevant clinical information. Participating in medication reconciliation and supporting safe transitions of care. Preparing, verifying, and overseeing sterile and non-sterile medication compounding as applicable. Providing oversight and guidance to pharmacy technicians and support staff. Responding to medication-related questions from physicians, nurses, patients, and other healthcare professionals. Participating in emergency response situations, including code blue and other critical patient events as required. Maintaining accurate medication records and documentation within the electronic medical record. Ensuring compliance with controlled-substance regulations, medication storage requirements, and hospital pharmacy policies. Assisting with medication inventory management, shortage mitigation, and formulary compliance. Identifying, documenting, and helping to prevent medication errors and adverse drug events. Providing patient and caregiver education regarding medications when appropriate. Preferred experience includes inpatient pharmacy operations, automated dispensing systems, sterile compounding, IV medications, antimicrobial stewardship, medication reconciliation, and electronic medication-management systems.
