Evaluate patients' conditions to ensure all issues are being treated. Develop effective medication plans that minimize the risk of adverse side effects as appropriate. Consult with prescribers, nurses, and patients on medications, doses, etc. Advise on correct administration of drugs. Assess the results and outcomes of pharmaceutical treatments. Collaborate with healthcare professionals to ensure optimal patient care. Keep accurate documentation of medication plans and patient progress. Consult regularly with healthcare team regarding appropriateness of orders, availability of medication, side effects, incompatibilities, formulary considerations, and current care path management. Participate in the education of students and residents as necessary. Description: The pharmacist position involves providing pharmaceutical care to patients. You will be responsible for reviewing patient records to ensure appropriate medication therapy. You will evaluate patient conditions and develop medication plans while minimizing risks. Collaboration with healthcare professionals is key to ensuring optimal patient care. Documentation of medication plans and assessments of treatment outcomes are essential.
