Evaluates and treats patients of all ages, modifying the plan of care as appropriate. Serves as a role model in the delivery of professional services and as a clinical resource for staff and students.

Required Qualifications:
 

• Bachelor's degree in Physical Therapy
• BLS certification (American Heart Association)
• Licensed Physical Therapist (PT) - Illinois Department of Financial and Professional Regulation (IDFPR)

• Demonstrates ability to perform patient transfers with assistance as needed
• Designs an appropriate plan of care integrating goals, treatment, outcomes, and discharge plan

• Communicates with referring providers on patient progress and treatment recommendations
