Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy services to residents within a skilled nursing facility setting
• Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
• Collaborate with interdisciplinary team members to coordinate patient care
• Educate patients and caregivers on therapeutic techniques and home exercise programs

Required Qualifications

• Active Occupational Therapist (OT) license in the state of North Carolina
• NBCOT certification required
• Experience working in a skilled nursing or long-term care setting
• Ability to evaluate, plan, and implement occupational therapy treatment programs

Preferred Experience / Skills

• Prior experience in a SNF or post-acute care environment
• Familiarity with MDS documentation and PDPM reimbursement models
• Strong communication and interdisciplinary team collaboration skills
