Key Responsibilities

• Conduct skilled home health visits for a diverse patient population including med/surg, cardiac, ortho, neuro, CVA, COPD, CHF, Parkinson's, trauma, and post-surgical cases
• Complete a minimum of 6–8 skilled home visits per day under Medicare, Medicare Advantage, and commercial insurance guidelines
• Perform and document OASIS assessments and visit notes using facility-designated electronic systems
• Self-schedule visits and hours based on the needs of the home care office, including 1–2 weekend shifts per contract
• Utilize provided equipment including laptop, MiFi, and portable printer to complete documentation in the field
• Assist patients with ADLs and functional rehabilitation goals across a broad range of diagnoses

Required Qualifications

• Active Florida or Compact Occupational Therapist license required at time of submission
• Minimum 1 year of home health experience required
• BLS certification required
• Valid driver's license and personal vehicle required to conduct home visits
• Must be COVID-19 vaccinated or have an approved exemption on file
• Must have flu shot or declination on file; mandatory mask wearing required at all times while on assignment
• Candidates must reside more than 100 miles from the facility to be eligible

Preferred Experience / Skills

• OASIS training or prior OASIS documentation experience preferred
• Cerner EMR experience preferred
• Experience working with Medicare and Medicare Advantage skilled home care populations
