Contract | Days | Monday–Friday, 8:00 AM – 5:00 PM | Onsite

Key Responsibilities

• Provide occupational therapy evaluations and treatments to patients in a home health setting
• Develop and implement individualized treatment plans to support patient recovery and functional independence
• Document patient progress accurately and in accordance with facility and regulatory standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning
• Maintain active Arizona OT license with no disciplinary actions on record
• Complete required learning modules prior to the start of the assignment

Required Qualifications

• Minimum of 24 months of occupational therapy experience, with experience within the last 12 months
• Active Arizona Occupational Therapist license, verified within the last 30 days, with no hits or disciplinary actions
• Active BLS certification from an approved vendor (AHA, ARC, or ASHI)
• Valid driver's license required to verify residency
• Must not be an Arizona resident — Arizona residents are not eligible for this assignment
• Completed OT skills checklist within the last 12 months

Preferred Experience / Skills

• Prior travel therapy experience highly preferred
• Experience working across multiple facility types, departments, or care settings
• Flexibility with locations, shifts, and departments to support placement efficiency
