Contract | 13-Week Assignment | Days | Monday – Friday, 8:00 AM – 5:00 PM | Onsite
Key Responsibilities
- Provide occupational therapy services to patients in a home health setting across an assigned territory
- Conduct evaluations and develop individualized treatment plans to support patient functional goals
- Deliver skilled OT interventions focused on improving daily living activities, safety, and independence
- Document patient progress and clinical notes using HCHB (HomeCare HomeBase) EMR
- Coordinate care with interdisciplinary team members to ensure continuity of patient care
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Washington
- Minimum of 12 months of Home Health experience within the past 3 years
- Proficiency or ability to work within HCHB (HomeCare HomeBase) EMR
- Valid driver's license and reliable transportation for home visits within assigned territory
Preferred Experience / Skills
- Familiarity with Spokane County and surrounding areas
- Experience managing a caseload across a multi-county territory
- Strong time management and independent practice skills in a field-based setting
