Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans in a post-acute care setting
• Implement therapeutic interventions to improve patients' functional independence in daily activities
• Monitor and document patient progress, adjusting treatment plans as clinically appropriate
• Collaborate with interdisciplinary care teams to ensure coordinated patient care
• Educate patients and families on home exercise programs and adaptive strategies

Required Qualifications

• Active Occupational Therapist (OT) license in the state of California
• NBCOT certification required
• Experience practicing as an Occupational Therapist in a clinical setting
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience in a post-acute or rehabilitation care environment
• Familiarity with documentation and EMR systems used in post-acute settings
• Strong interpersonal and communication skills for working within multidisciplinary teams
