Key Responsibilities
- Conduct occupational therapy evaluations and revisits for home health patients across the assigned service area
- Complete an average of four patient visits per day, including a combination of initial evaluations and follow-up revisits
- Provide skilled occupational therapy services to patients in their homes within the northeastern Sacramento, Folsom, and Woodland/Yolo County service areas
- Maintain accurate and timely clinical documentation in accordance with home health standards
- Travel independently to patient homes throughout the designated coverage area using a personal vehicle
Required Qualifications
- Active California Occupational Therapist (OT) license
- Minimum of 2 years of experience in a healthcare setting
- Current Basic Life Support (BLS) certification
- Active National Provider Identifier (NPI) number
- Valid driver's license and proof of current automobile insurance
Preferred Experience / Skills
- Prior home health experience strongly preferred
- Certified Occupational Therapy Assistant (COTA) with home health experience may be considered in lieu of OT applicants
