Key Responsibilities
- Provide occupational therapy services across a split setting: approximately 50% Home Health and 50% Skilled Nursing Facility, with flexibility based on area need
- Utilize setting-specific software platforms for documentation and care management
- Meet productivity expectations established for each care setting
- Collaborate with an onsite Therapy Supervisor and interdisciplinary care team to support patient outcomes
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Minnesota
- Ability to work across both Home Health and Skilled Nursing Facility settings as needed
- Proficiency or ability to learn EMR platforms (HCHB for Home Health; NetHealth for SNF)
- Must reside within 80 miles of the facility to be considered a local placement
Preferred Experience / Skills
- Prior experience in Home Health and/or Skilled Nursing Facility settings
- Strong adaptability and flexibility to shift between care environments based on patient and facility need
- Familiarity with productivity standards in therapy-based settings
