Key Responsibilities
- Provide occupational therapy services to patients across multiple care locations
- Evaluate patients and develop individualized treatment plans to support functional independence
- Implement therapeutic interventions in alignment with facility protocols and patient goals
- Coordinate care and communicate effectively across multiple facility settings
- Document patient progress and maintain accurate clinical records in accordance with regulatory standards
Required Qualifications
- Active Occupational Therapist (OT) license in the state of Illinois
- Graduate of an accredited Occupational Therapy program
- NBCOT certification (or eligibility)
- Ability to work across multiple facility locations as required
Preferred Experience / Skills
- Prior experience working in a multi-site or split-facility setting
- Strong organizational and time management skills to manage caseloads across locations
