Key Responsibilities

• Provide occupational therapy services to patients in a home health setting
• Evaluate patients and develop individualized treatment plans to support functional recovery and independence
• Deliver skilled OT interventions aligned with physician orders and care team goals
• Document patient progress, treatment plans, and clinical notes in accordance with regulatory and facility standards
• Collaborate with interdisciplinary care teams to coordinate patient care and discharge planning

Required Qualifications

• Minimum of 24 months of Occupational Therapy experience in a Home Health setting, with experience within the last 12 months
• Active Arizona Occupational Therapist license, verified within the last 30 days, with no disciplinary actions or hits on record
• Active BLS certification from an approved vendor (AHA, ARC, or ASHI)
• Valid driver's license required to verify non-Arizona residency
• OT Home Health skills checklist completed within the last 12 months
• Must be available to start on the assigned start date or within one week of that date

Preferred Experience / Skills

• Prior travel therapy assignment experience highly preferred
• Flexibility with locations, shifts, and departments is strongly advantageous
