    Job Title: Occupational Therapist Profession: Occupational Therapist Specialty: Home Health Duration: Full-time Shift: Monday - Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: One year experience caring for a similar patient population is required. License: Licensed as an Occupational Therapist in the state of practice. Certifications: Valid Basic Life Support (BLS) Healthcare Provider Card. Must-Have: Valid driver’s license with automobile insurance as per state requirements. Education from an accredited occupational therapist education program. Eligibility or completion of the entry-level certification examination for Occupational Therapists. Description: As an Occupational Therapist in our Home Health team, you will bring your clinical expertise and compassionate care directly to patients in their homes. You will play a key role in developing and delivering personalized therapy plans. The goal is to help patients regain independence, improve their quality of life, and achieve their goals. Working closely with physicians and the care team, you will provide hands-on treatment, education, and support. All your actions will be grounded in best clinical practices. Your impact will be seen in improved patient outcomes and meaningful relationships with the individuals you serve. Responsibilities include visiting patients' homes, conducting ongoing interdisciplinary assessments, and evaluating patients' levels of functioning. You will also develop specific occupational therapy procedures and functional activities in accordance with treatment plans. Overseeing the use of treatment modalities, prosthetic devices, and therapeutic appliances will be part of your role.
