    Key Responsibilities
    • Contribute professional and clinical knowledge as a member of an interdisciplinary therapy team providing home health services
    • Manage a caseload of approximately 4–6 patients per day, primarily patients requiring short-term therapy following hospitalization or nursing home stays
    • Travel within approximately 50 miles of the Fort Dodge area to conduct home visits, averaging 200–250 miles per week
    • Document patient care and progress using EPIC EMR
    • Participate in weekly virtual team meetings and monthly in-person team meetings
    Required Qualifications
    • Minimum 2 years of occupational therapy experience
    • Active Iowa state OT license
    • Current BLS certification
    • Must possess a valid driver's license and personal vehicle (mileage reimbursed)
    • Must maintain a taxable home address located more than 50 miles from the assignment location
    Preferred Experience / Skills
    • Prior home health experience (department is willing to train candidates without it)
    • Experience with EPIC EMR
    • First-time travelers are welcome to apply
