Key Responsibilities
Contribute professional and clinical knowledge as a member of an interdisciplinary therapy team providing home health services
Manage a caseload of approximately 4–6 patients per day, primarily patients requiring short-term therapy following hospitalization or nursing home stays
Travel within approximately 50 miles of the Fort Dodge area to conduct home visits, averaging 200–250 miles per week
Document patient care and progress using EPIC EMR
Participate in weekly virtual team meetings and monthly in-person team meetings
Required Qualifications
Minimum 2 years of occupational therapy experience
Active Iowa state OT license
Current BLS certification
Must possess a valid driver's license and personal vehicle (mileage reimbursed)
Must maintain a taxable home address located more than 50 miles from the assignment location
Preferred Experience / Skills
Prior home health experience (department is willing to train candidates without it)
Experience with EPIC EMR
