Key Responsibilities

• Provide occupational therapy services to patients within a home health setting across a 50+ mile radius of the assigned office
• Meet a minimum productivity expectation of 30 points per week
• Complete all clinical documentation by 10:00 AM the following day
• Utilize HCHB (HomeCare HomeBase) for documentation and care coordination

Required Qualifications

• Valid Occupational Therapist (OT) license
• Minimum of one (1) year of home health experience required
• Proficiency with HCHB (HomeCare HomeBase) EMR system
• Ability to travel within a 50+ mile radius of the assigned office

Preferred Experience / Skills

• Strong time management and self-directed work habits suited for a field-based role
• Experience meeting productivity benchmarks in a home health environment
