Key Responsibilities
Evaluate and treat patients within a Skilled Nursing Facility (SNF) setting to restore or improve functional independence
Develop and implement individualized occupational therapy treatment plans based on patient needs and goals
Document patient progress, treatment plans, and outcomes in accordance with facility and regulatory standards
Collaborate with interdisciplinary care teams to coordinate comprehensive patient care
Educate patients and family members on therapeutic techniques and home exercise programs
Required Qualifications
Graduate of an accredited Occupational Therapy program
Current and valid Occupational Therapist (OT) licensure in the state of California
Current CPR/BLS certification
Experience working in a Skilled Nursing Facility (SNF) setting
Preferred Experience / Skills
Prior experience with geriatric patient populations
Familiarity with MDS documentation and Medicare guidelines in a SNF environment
